I authorize Berndt & Associates, P.C. to debit my bank account indicated above for the above indicated amount. I certify that I am authorized to initiate transactions on this account. I understand that Berndt & Associates, P.C. assesses a $1.00 (one dollar) fee for use of this ACH payment service and that the processing fee will be deducted from this total payment prior to posting to my account. As such, my account will be credited by the amount of this ACH payment less processing fees assessed by Berndt & Associates, P.C. I further acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
